Volunteer
PHE would not be what it is without all the incredible parents/community support that we have. Volunteers help out all over campus - in class, hosting stations, during weekly specials (like Art or Library), and (most enjoyably!) at lunch, supporting supervision. Cleared volunteers are also able to chaperone field trips.
PLEASE FOLLOW THESE INSTRUCTIONS IN ORDER TO BE CLEARED TO VOLUNTEER IN OUR CLASSROOMS OR ATTEND FIELD TRIPS
- Contact your Physician for Tuberculosis (TB) clearance
- Complete our VOLUNTEER REQUEST FORM
- Complete the LIVESCAN FORM for an outside agency.

Take the Livescan form to the outside agency of your choice and complete the fingerprinting process. The agency will charge a fee for this service. The results will be sent electronically to the district office.
Email your TB clearance, volunteer request form, and Livescan form to the MDUSD district office at volunteers@mdusd.org. Or you may bring the paperwork to Human Resources at the district office, 1936 Carlotta Drive, Concord.
Once the District Office has received the above requested documents from you and your fingerprints have been cleared through the Dept of Justice (DOJ), the District will email you that you have been cleared to volunteer. Once you receive this email, please email the PHE office to join a volunteer orientation with the principal. It is a short, 15-minute Zoom meeting to go over the school rules.
